Cariology

(From Sturdevants art & science of operative dentistry 4th edition)

1.       Polarization of DC – limited segment of population experiences most of disease . 80% DC in 20% children from lower socioeconomic status
2.       MC chronic childhood disease – DC
3.       DC causing organisms – S mutans – 8 strains a to h, S cricetus, S rattus , S ferus, S sobrinus
4.       Most imp factor in causing DC is – high frequency sucrose exposure
5.        Plaque formation
a)      Acquired pellicle forms within 2 hours of brushing (30 mins -1 hr)
b)      12-24 hrs- cocci
c)       1-3days –cocci +filamentous bacteria
d)      1wk – palisades of cocci + filamentous bacteria
e)      3 wks – “corncorb”

6.       Colonizations
Dorsum of tongue
Micrococcus mucilaginous, S salivarius
Teeth
S mutan , S mitis
Non carious teeth
S sanguis
Buccal mucosa
S mitis

7.       Caries producing organisms – anerobic, ph <5.5 & negative redox potential
8.       Combination of food and shelter- ‘ecological niche’
9.       Niche saturation leads to ‘colonization resistance’
10.   “Bacteriocin” produced by S mutans leads to colonization resistance
